When I was twelve, my sister and I accompanied my grandparents to their annual yoga retreat in the hilly ranges of southern India. We had never been before, but the summer heat was particularly blistering that year, so we persuaded our grandparents to take us along. I envisioned a blissful two-week vacation in a photogenic little hamlet, nestled among tea plantations, in temperatures that were thirty degrees cooler than on the mainland. It was just that, and yet, it was even less complicated than that.
The mean age of folk at the retreat was 67. Bells sounded every morning at 3:30am, and everyone filed out of their cabins and to a little gymnasium in the center of the dwelling, where we all meditated for two hours to the sounds of sitar music and transcendental humming. Meals were served at strict hours three times a day, and consisted of boiled vegetables and grains, with not a lick of salt or spice. The library in the middle of this utopian dwelling held only spiritual and philosophical texts, not the Nancy Drew or Hardy Boys I hoped for. In the afternoons, there were a variety of classes offered ? cooking lessons, devotional classes, music and instrumental classes, and yoga. My sister and I stopped by the latter occasionally, and were always put to shame by octogenarians holding themselves up in impossible poses, such as balancing their entire habitus on the tips of their fingers. I journaled in the evenings, writing each day about a new facet of human life that I?d observed. In the absence of stimulus, my dreams grew vivid and exceedingly detailed.
Over the course of the two weeks, my sister and I grew quiet and reflective. It was then that we began an important switch in our minds, something that has lasted to this day. We began to see value in living leanly, economically, functionally. We began to separate needs from wants, and to discard the ornamentation.

Third year of medical school has finally brought me around to my surgery rotation: three months of waking up at 4am, stuffing my white coat pockets to the brim with gauze and tape, retracting skin and fat during long abdominal surgeries, and practicing suturing techniques on pig?s feet procured at the local Stop&Shop grocery market. It?s fast and exhilarating, and deeply satisfying. I was skeptical when I first heard that my preceptor?s favorite procedure of all time was draining a deep-seated abscess. But when I saw it being done in clinic, how a single stab of the thing blade led to the gushing of what felt like liters of pus, I couldn?t help but agree. What a joy to just go in and fix a problem so dramatically, reconstruct a failing human body in a matter of hours!
During orientation on the first day of the rotation, two residents sat down and gave my classmates and me some hard advice. Surgery is a demanding rotation, they said, and it reflects the demanding residency ahead that awaits the select few. We could expect to go in while it is still dark out, and leave after the sun had set, almost every day. Residents and attendings can be rough around the edges, and may be gruff with you, even kick you out of their operating room if they feel like it, but it?s not personal. Or even if it is, we?ve got to shrug it off and keep it moving. Gone are the days of noon conferences and luxurious afternoon didactics, with their promise of free lunches and coffee. We were to eat when we can, sit when we can, sleep when we can.
After an hour of such grim prognostications, my classmates and I took a break and debriefed our feelings with each other outside the bathrooms. Some were giggling nervously with panicked eyes, but most looked inspired. I too felt like I had voluntarily signed up for a warrior training program, and was feeling pretty zen about it. I saw it as a character-building experience: surgery was the time to cut out the silly frills, and embrace a leaner, meaner way of living. It was time to lose the pretty business casual outfits and fancy footwear of internal medicine, and trade them in for utilitarian scrubs and clogs. It promised to be a time of talking less and getting things done.

For instance, Toyota emphasizes innovation on the shop floor by frontline workers to solve problems in production in real time. If a problem is discovered that cannot be fixed within the production cycle time, workers pull a cord that halts the entire assembly line and brings a senior supervisor to the scene. The management aggressively seeks ideas for improvement from employees, resulting in an average yield of close to a million ideas annually, 90% of which go on to be implemented.
Analysts attribute Toyota?s success to its emphatic optimization of flow ? information flow, physical flow of parts, and overall production flow, via standardized processes and continuous improvement. Standardized processes are ones that are streamlined to eliminate aberrations and unplanned redundancies. Waste, measured even in the seconds, is simply not tolerated, forcing a redesign of processes, again and again, which any employee can take on.
In 2004, Toyota surpassed Ford Motors to become the world?s second-largest manufacturer of cars and trucks, surpassing the latter consistently in quality, dependability and value assessments. In turn, Ford began to take cues from Toyota, transforming its assembly-line system to similarly cut out waste.

They?ve been teaching me to appreciate the vulnerabilities of the human body ? for no matter how exhausted or sleep-deprived I may feel, actually laying hands on the more tangible deficits of another?s is always startling and humbling. The end result is a beautiful dance, for surgeons and their assistants, working with their hands, rediscovering the grace of human movement, bring art back into medicine.
I never leave the OR thinking that more is better. I watch instruments fly, I watch the players push and pull, cut and stitch, wash and dry, and I think about things like symmetry, precision, and above all, the beauty of economy.

Does it seem like courts keep telling us we have to monitor all juvenile behavior in the workplace? Do we need to hire ?conduct cops? for our hallways? A court ruling last week smacked some sense into the laws and said ?no? ? not all bad-boss behavior is automatically ?discrimination.? Is this an early holiday gift to employers?
Case in Point: Josalynn Brown and Carolyn Wilson, who are African-American, worked as nurses at a suburban Chicago hospital. They joined 10 other nurses in complaining to HR that Filipino nurses were receiving better assignments and training than black nurses. Hospital officials investigated, but couldn?t corroborate the claims.
Eventually, Brown and Wilson requested and were granted a transfer to another hospital owned by the parent company. But the complaints didn?t stop. Brown and Wilson voiced several protests to HR, including that their new supervisors were giving them the ?cold shoulder? treatment and had engaged in mild name-calling, referring to one of the nurses as a ?cry baby? and a ?spoiled child.?
Brown and Wilson again asked to be switched to another location. This time, the request was denied. They sued the hospital for race discrimination and retaliation.
The verdict: Title VII of the federal Civil Rights Act makes it unlawful to discriminate against employees based on their race with respect to their ?compensation, terms, conditions or privileges of employment.? But receiving the ?cold shoulder? from a boss and mild name-calling does not rise to the level of actionable discrimination, the court said.
?The fact that someone disagrees with you (or declines to take your advice) does not, without more, suggest that they discriminated against you,? the court ruled. ?Title VII protects against discrimination, not ?personal animosity or juvenile behavior.' Personality conflicts at work that generate antipathy and ?snubbing by supervisors and co-workers' are not actionable under Title VII.? (Brown v. Advocate S. Suburban Hosp., 7th Cir., 11/21/12)
3 Lessons Learned ? Without Going to Court
1. Investigate immediately. The court noted that the hospital responded to the nurses race and retaliation discrimination complaints right away with an investigation. HR tried to connect the dots to see if the new supervisor?s conduct was motivated by the nurses? previous discrimination complaints. However, the supervisors had no knowledge of the nurses? previous complaints at the prior location, so no racial motivation could be shown.
2. Prohibit name calling as a whole. Even though all name-calling behavior may not be illegal (the court noted the name-calling in this case made no references to race), it may create an unpleasant culture to work in. Even though the court said Title VII doesn?t protect ?personal animosity or juvenile behavior,? name-calling can still cost big bucks to defend.
3. Make your harassment, discrimination and retaliation prevention policy stricter than the law. The prohibited conduct listed in your Harassment, Discrimination and Retaliation Prevention Policy should be stricter than what the law will allow in order to curtail these kinds of lawsuits. That being said, remember to include language like, ?A violation of this policy does not necessarily amount to a violation of the law.? This will go far in showing that just because your culture will not tolerate certain behaviors it does not make engaging in them illegal in the eyes of the law.

BEIRUT (AP) ? Syrian rebels captured a helicopter base just outside Damascus Sunday in what an activist called a "blow to the morale of the regime" near President Bashar Assad's seat of power.
The takeover claim showed how rebels are advancing in the area of the capital, though they are badly outgunned, making inroads where Assad's power was once unchallenged. Rebels have also been able to fire mortar rounds into Damascus recently.
The director of the Britain-based Syrian Observatory for Human Rights, Rami Abdul-Rahman, said rebels seized control of the Marj al-Sultan base on the outskirts of Damascus on Sunday morning. He said at least 15 rebels and eight soldiers were killed in the fighting that started a day earlier. The rebels later withdrew from the base.
Rebels appear to be trying to take over air bases and destroy aircraft in order to prevent the regime from using them in attacks against opposition forces around the country.
The rebels have no protection against the attack helicopters and fighter jets that have been blasting their positions.
Rebels have been attacking air bases in different parts of Syria, mostly in the northern regions of Idlib and Aleppo.
In the battle at the base outside Damascus, Abdul-Rahman and Damascus-based activist Maath al-Shami said rebels destroyed two helicopters with rocket propelled grenades and captured a tank. They say the base, which is on the eastern outskirts of Damascus, houses several radar positions.
"This is a blow to the morale of the regime, because it is close to the heart of the capital," said Abdul-Rahman, referring to the base that is about 15 kilometers (10 miles) from Damascus.
Al-Shami said the rebels withdrew from the base after they captured some ammunition. He said they feared counterstrikes by regime aircraft.
An amateur video posted online showed rebels walking next to two destroyed helicopters. At least three other helicopters appeared undamaged. Black smoke billowed in the distance.
Another video showed several radar posts on hills inside the large compound. Parked military trucks stood inside as rebels roamed freely.
The activist videos appeared genuine and corresponded to other AP reporting about the events depicted. Syria restricts the access of reporters.
The Observatory also reported violence in other parts of Syria, including the country's largest city of Aleppo in the north and the capital itself.
It said rebels on Sunday captured a training base for the Popular Front for the Liberation of Palestine-General Command near the Damascus suburb of Douma. The PFLP-GC is one of the Palestinian factions most loyal to Assad.
The PFLP-GC said in a statement late Saturday that the base was under attack. It said that thousands of activists and fighters who fought against Israel were trained at the base over the past 30 years.
Also Sunday, the Observatory said a bomb targeted a bus in the southern village of Othman, killing at least five people and wounding dozens. It said rebels and troops clashed in the southern region of Quneitra on the edge of the Israeli-occupied Golan Heights.
The Local Coordination Committees, another activist group, said residents found 12 bodies in the Damascus suburb of Daraya, scene of heavy clashes between rebels and government troops over the past few days.
State TV said troops clashed with al-Qaida militants in Daraya, killing some of them and confiscating a mortar that they were using in their attacks.
The station said that troops killed an al-Qaida affiliated Palestinian militant known as Abu Suhaib in the Damascus suburb of Hajira. It said his group was behind several bombings in Syria that killed and wounded dozens of people.
Assad's regime blames the revolt on a foreign conspiracy. It accuses Saudi Arabia and Qatar, along with the United States, other Western countries and Turkey, of funding, training and arming the rebels, whom it calls terrorists.
Syria's conflict erupted in March 2011 with an uprising against President Bashar Assad's regime, inspired by other Arab Spring revolts. It quickly morphed into a civil war that has since killed more than 40,000 people, according to activists.

Now that Congress and the Obama administration are returning to Washington, we know they'll be looking over a fiscal cliff. But the skies above that landscape look different since we met Superstorm Sandy the week before the election.?That punishing storm reminded us, as Business Week screams on its cover, "It's Climate Change, Stupid."
A single storm isn't "climate change." But then, we aren't talking about single events any more. Sandy was part of a growing and disturbing pattern, whether it's called Sandy or Katrina or Snowmageddon or a nationwide heat wave that broke 3,215 high temperature daily records this past June.
By overwhelming numbers, Americans want to tackle the urgent conservation and environmental issues facing our coasts, our air and water, and our birds, wildlife and natural places.
How do we know that? In our grassroots campaign calling for an end to the partisanship that has paralyzed political action on conservation and the environment, the National Audubon Society and our Republican partner, ConservAmerica, asked folks a critical question that was never posed in any of the presidential debates:
What should be the top conservation and environmental priorities for the next administration?
Democrats, Republics and independents responded from across the country. Their answers and the priorities they assigned offer a valuable roadmap for President Obama, the Congress and other elected officials.
No. 1. Promote the development of clean, renewable energy sources to reduce dependence on fossil fuels and foreign sources.
This was by far the most frequently listed priority, from writers in Hawaii to bloggers in New York City. Debrah Roemisch echoed the concerns of many when she wrote, "Developing clean energy sources will help the environment, provide jobs and help us be independent..."
No. 2: Protect air, water and land from pollution.
"Clean air and water...Take the politics out," demanded Corrine Carter of Prattville, Ala. In comment after comment, folks across the country expressed dismay that conservation - once one of the great unifying issues in America - has become a victim of drive-by partisan politics.
No. 3. Be cautious with the Keystone XL pipeline, offshore drilling and hydrofracking in environmentally sensitive areas.
"Drop forever the Keystone pipeline project and replace it with a national smart grid (a heck of a lot more jobs,)" wrote Mark Ford.
"Put a moratorium on fracking in the U.S." said Rosann Strum of Bloomington, Ind. "Stop deep water drilling."
We believe our elected officials can find common sense solutions for well-managed energy development that protects sensitive areas like the Arctic while helping to meet America's energy and employment needs.
No. 4: Protect national and state parks and open spaces.
"We need to restore and keep full protections for our wilderness, wildlife, national parks," said Barbara Eaton of Allenstown, N.H., expressing the concerns of many respondents.
Just as Hurricane Katrina before her, Superstorm Sandy demonstrated the short-sightedness of draining, paving and building atop our natural storm barriers - marshes, seashores and other wetlands.
Without the protections nature provided, the storms slam into populated shorelines full force with no buffers to slow winds or water surges.
No. 5: More environmental and conservation education.
Educating our youth to care for the communities and the planet they will inherit leave to their children was a recommendation repeated multiple times.
A strong common thread linked the environmental priorities Americans offered the next administration and Congress. In message after message, Americans of every political stripe said they were fed up with the do-nothing partisan politics that has infected virtually every environmental issue.
Ms. Eaton from New Hampshire summed up the real challenge for America's elected leaders: "Both parties must realize that our Earth and wildlife are not battlegrounds."

GAZA CITY, Gaza Strip (AP) ? A leading Islamic cleric in the Gaza Strip has ruled it a sin to violate the recent cease-fire between Israel and the Hamas militant group that controls the Palestinian territory.
The fatwa, or religious edict, issued by Suleiman al-Daya late Saturday accords a religious legitimacy to the truce and could justify any act by Gaza's government to enforce it.
"Honoring the truce, which was sponsored by our Egyptian brethren, is the duty of each and every one of us. Violating it shall constitute a sin," the fatwa read.
The Wednesday truce put an end to an eight-day Israeli offensive against Gaza militants who fired rockets into Israel. The agreement remains fragile because details beyond the initial cease-fire have not yet been worked out.

There is a service that you can turn to that will automatically delete comment and blog spam so that you don?t have to worry about it anymore.? The service is called Akismet, and you can access it through plugins such as WordPress or the Akismet API.? All you have to do is register and API key (like a password) which is your own private access to your account.
Then, each time a comment, pingback or trackback?appears on your site, the Akismet service tests it and either Okays it or turns it down.? I?ve spoken about ?black hat techniques? before, and web spamming is definitely one of the worst.? They?re hoping you?ll disregard them because these spam techniques use flattery, praise and other positive comments to catch you off guard.
There?s no way for you to know if these comments and posts are legitimate.? That?s why it?s a good thing to use a plug in like Akismet to check it out for you.? Just be sure you?re protected from these spammers who are thieves and who can run your business off the Internet.?

SAN JUAN, Puerto Rico (AP) -- Hector ''Macho'' Camacho was a brash fighter with a mean jab and an aggressive style, launching himself furiously against some of the biggest names in boxing. And his bad-boy persona was not entirely an act, with a history of legal scrapes that began in his teens and continued throughout his life.
The man who once starred at the pinnacle of boxing, winning several world titles, died Saturday after being ambushed in a parking lot back in the Puerto Rican town of Bayamon where he was born. Packets of cocaine were found were found in the car in which he was shot.
Camacho, 50, left behind a reputation for flamboyance - leading fans in cheers of ''It's Macho time!'' before fights - and for fearsome skills as one of the top fighters of his generation.
''He excited boxing fans around the world with his inimitable style,'' promoter Don King told The Associated Press.
Camacho fought professionally for three decades, from his humble debut against David Brown at New York's Felt Forum in 1980 to an equally forgettable swansong against Saul Duran in Kissimmee, Florida, in 2010.
In between, he fought some of the biggest stars spanning two eras, including Sugar Ray Leonard, Felix Trinidad, Oscar De La Hoya and Roberto Duran.
''Hector was a fighter who brought a lot of excitement to boxing,'' said Ed Brophy, executive director of International the Boxing Hall of Fame. ''He was a good champion. Roberto Duran is kind of in a class of his own, but Hector surely was an exciting fighter that gave his all to the sport.''
Camacho's family moved to New York when he was young and he grew up in Spanish Harlem, which at the time was rife with crime. Camacho landed in jail as a teenager before turning to boxing, which for many kids in his neighborhood provided an outlet for their aggression.
''This is something I've done all my life, you know?'' Camacho told The Associated Press after a workout in 2010. ''A couple years back, when I was doing it, I was still enjoying it. The competition, to see myself perform. I know I'm at the age that some people can't do this no more.''
Former featherweight champion Juan Laporte, a friend since childhood, described Camacho as ''like a little brother who was always getting into trouble,'' but otherwise combined a friendly nature with a powerful jab.
''He's a good human being, a good hearted person,'' Laporte said as he waited with other friends and members of the boxer's family outside the hospital in San Juan after the shooting. ''A lot of people think of him as a cocky person but that was his motto ... Inside he was just a kid looking for something.''
Laporte lamented that Camacho never found a mentor to guide him outside the boxing ring.
''The people around him didn't have the guts or strength to lead him in the right direction,'' Laporte said. ''There was no one strong enough to put a hand on his shoulder and tell him how to do it.''
George Lozada, a longtime friend from New York who flew to Puerto Rico on Saturday, recalled that just hours after he was released from prison after serving a murder sentence, he received a call from Camacho, who was waiting outside his apartment in a black Porsche.
''He said, 'Come down, I'm taking you shopping,''' Lozada said, wiping away tears.
''Because of him, man, I got what I got today,'' he said, pointing to pictures on his smartphone of his 6-year-old daughter. ''Because of Hector, I stopped the drug scene ... He's helped so many people.''
Drug, alcohol and other problems trailed Camacho himself after the prime of his boxing career. He was sentenced in 2007 to seven years in prison for the burglary of a computer store in Mississippi. While arresting him on the burglary charge in January 2005, police also found the drug ecstasy.
A judge eventually suspended all but one year of the sentence and gave Camacho probation. He wound up serving two weeks in jail, though, after violating that probation.
Camacho's former wife, Amy, obtained a restraining order against him in 1998, alleging he threatened her and one of their children. The couple, who had two children at the time, later divorced.
He divided his time between Puerto Rico and Florida in recent years, appearing on Spanish-language television as well as on a reality show called ''Es Macho Time!'' on YouTube.
Inside the boxing ring, Camacho flourished. He won three Golden Gloves titles as an amateur, and after turning pro, he quickly became a contender with an all-action style reminiscent of other Puerto Rican fighters.
Long promoted by Don King, Camacho won his first world title by beating Rafael Limon in a super-featherweight bout in Puerto Rico on Aug. 7, 1983. He moved up in weight two years later to capture a lightweight title by defeating Jose Luis Ramirez, and successfully defended the belt against fellow countryman Edwin Rosario.
The Rosario fight, in which the victorious Camacho still took a savage beating, persuaded him to scale back his ultra-aggressive style in favor of a more cerebral, defensive approach.
The change in style was a big reason that Camacho, at the time 38-0, lost a close split decision to Greg Haugen at Caesar's Palace in Las Vegas in 1991.
Camacho won the rematch to set up his signature fight against Mexico's Julio Cesar Chavez, this time at the Thomas & Mack Center in Las Vegas. Camacho was roundly criticized for his lack of action, and the Mexican champion won a lopsided unanimous decision to retain the lightweight title.
''Even though people say I beat him easily, it wasn't that way,'' Chavez told Mexico's ESPN-Radio Formula this week. ''He was a very fast fighter, he faced everything and it was very hard for me.''
''He revolutionized boxing, Chavez said. ''It's a shame he got mixed up in so many problems.''
After that loss, Camacho became the name opponent for other rising contenders, rather than the headliner fighting for his own glory.
He lost a unanimous decision to another young Puerto Rican fighter, Trinidad, and was soundly defeated by De La Hoya. In 1997, Camacho ended Leonard's final comeback with a fifth-round knockout. It was Camacho's last big victory even though he boxed for another decade.
The fighter's last title bout came in 1997 against welterweight champion Oscar De La Hoya, who won by unanimous decision. Camacho's last fight was his defeat by Saul Duran in May 2010. He had a career record of 79-6-3.
Doctors pronounced Camacho dead on Saturday after he was removed from life support at his family's direction. He never regained consciousness after at least one gunman crept up to the car in a darkened parking lot and opened fire.
No arrests and have been made, and authorities have not revealed many details beyond the facts that police found cocaine in the car and that the boxer and his friend, who was killed at the scene, had no idea the attack was coming. ''Apparently, this was a surprise,'' said Alex Diaz, a police spokesman.
Survivors include his mother; three sisters, Raquel, Estrella and Ester; a brother, Felix; and four sons, Hector Jr., Taylor, Christian and Justin.
